Revision Date: December 2025 POSITION SUMMARY: This position
coordinates total nursing care for patients, participates in
patient and family teaching, acts as a patient advocate and works
as part of a team with ancillary and non-nursing departments.
ESSENTIAL FUNCTIONS: To perform this job, an individual must
perform each essential function satisfactorily with or without
reasonable accommodation. - Assesses the patient's condition and
nursing care needs. - Sets goals and prescribes appropriate nursing
interventions to meet physical, psychological, social and
rehabilitative needs of patients. - Develops individual nursing
care plans. - Implements care plans and makes revisions as needed.
- Evaluates patient care outcomes. - Collaborates with physicians
and other hospital employees. - Administers medications, treatments
and therapies. - Communicates with patients and oversees provision
of care. - Provides patients and/or family with instruction and
education relevant to condition. - Accurately documents nursing
observations, assessments, medications, treatments, and other care
given. - Regular and reliable attendance. - Perform other duties as
assigned. Additional Information: - Maintains facility equipment,
supplies and work areas. - Assists in the orientation of new
employees. - Coordinates and supervises activities of employees and
department operations. - Participates in performance/process
improvement activities. KNOWLEDGE, SKILLS & ABILITIES: The

combination of education, experience, and training that provides
the required knowledge, skills, and abilities. CERTIFICATIONS /
LICENSES: Current Apprentice Registered Nurse license in the state
